Giant mucocele of the colon at the distal stump due to low-grade mucinous neoplasia

چکیده
We present the first ever report on a colonic mucocele observed at the distal stump of a transverse loop colostomy caused by neoplasia. A 37-year-old female consulted us because of abdominal pain and vomiting caused by cystic lesions in the upper left abdominal quadrant. A preoperative checkup revealed no sign of neoplastic lesions; however, tumor resection was performed because of the symptoms. The tumor was a mucocele of the distal stump of the transverse colon with obstruction interposed between the mucocele and stoma. Pathological diagnosis was low-grade adenoma; however, it appeared like low-grade mucinous neoplasia of the appendix rather than a normal colonic adenoma. The neoplasia existed in the transitional segment between obstruction and dilatation. As this is the first case of colonic mucocele caused by mucinous neoplasia, no definite consensus for diagnosis and treatment exists. With reference to low-grade mucinous neoplasia, we propose that complete surgical resection be performed for diagnosis and a favorable outcome.

BACKGROUND Mucinous cystadenocarcinoma is the second most common etiology of appendiceal mucocele. We report a relatively rare case of a giant appendiceal mucocele caused by mucinous cystadenocarcinoma, which occupied the entire abdomen of an adult woman. CASE PRESENTATION A 63-year-old woman presented with a chief complaint of abdominal distention. Imaging studies showed a giant cystic mass ...

Mucocele of the appendix is an uncommon disorder that is often asymptomatic, but can present similarly to acute appendicitis. Timely diagnosis and treatment is imperative due to the many complications that can result from the mucocele, such as perforation. Appendiceal mucoceles (AM) were previously thought to be either benign or malignant; however, a different pathological classification of AM ...
متن کاملMucocele of the appendiceal stump due to benign mucinous cystadenoma.
Mucocele of the appendix is a rare lesion, characterized by distension of the lumen due to accumulation of mucus material. Correct preoperative diagnosis is seldom achieved. If left untreated, the mucocele may rupture producing a potentially fatal peritoneal spread. The type of surgical treatment is related to the dimensions and histology of the mucocele. In this paper, the case of a 49-year-ol...
متن کاملGiant colonic mucocele following palliative surgery for metastatic adenocarcinoma
We report an unusual case of a giant colonic mucocele following ileo-sigmoid bypass surgery in a patient with advanced adenocarcinoma of the splenic flexure. The formation of a giant colonic mucocele resulted from distal splenic flexure obstruction due to tumour relapse and proximal caecal obstruction due to peritoneal disease with subsequent accumulation of mucus in the closed loop.
